Initial orthostatic hypotension (IOH) is one type of OH and it is defined as a fall in systolic blood pressure (BP) of > 40 mmHg and/ or > 20 mmHg in diastolic BP on upright position within 15 seconds. Symptoms of OH include light-headedness, visual blurring, dizziness, and fatigue. In OH patients, abnormal eye movements including downbeat and periodic alternating nystagmus were reported. Here in, we report a 24-year-old man who had ictal miosis in IOH with decreased serum level of norepinephrine. We assessed relationship between pupil diameter and vital sign such as systolic pressure, diastolic pressure and heart rate.
